You'll provide whole-family primary care in the home setting, managing a panel of patients through longitudinal care. You'll collaborate with an interdisciplinary team of physicians, behavioral health counselors, and dieticians. 🎯 What You'll Do

• Manage a panel of families as primary care provider
• Provide longitudinal primary and preventive care in home and via telehealth
• Collaborate with interdisciplinary team including MDs and LCSWs
• Order, administer, and record diagnostic tests

📋 Requirements

• Completion of NP or PA accredited program and active Arizona license
• 2+ years experience providing clinical services to pediatric and adult patients
• Proficiency in clinical nursing skills like venipuncture and patient assessments
• Full-time availability with rotating call schedule including evenings and weekends
