💼 About This Role
You'll serve as Waymark's frontline presence in DeKalb County, connecting Medicaid patients to social support and resources as part of a multidisciplinary care team. Your core impact will be helping patients overcome barriers like homelessness and food insecurity to achieve better health outcomes.
🎯 What You'll Do
- Conduct needs assessments with patients in community, home, and healthcare settings.
- Help patients address social issues like homelessness, substance use, and hunger.
- Build and maintain a directory of community resources (e.g., food banks, housing).
- Document patient encounters in technology systems and participate in weekly care huddles.
📋 Requirements
- Highly organized and self-motivated to work independently.
- Reside within a commutable distance of DeKalb County, Illinois.
- Available to work at least one weekend day per week and one late-start shift.
- Travel required up to 80% within surrounding counties; valid driver's license and insured vehicle.
