Description

You will provide psychosocial services to terminally ill patients and their families, including assessments, counseling, and referrals. You'll collaborate with an interdisciplinary team to develop and coordinate care plans.

Requirements

• Master's degree in social work from CSWE-accredited school
• Active Ohio Social Work license
• Valid driver's license, reliable insured transportation
• Minimum 1 year healthcare experience with terminally ill patients
• Effective communication and assessment skills

Responsibilities

• Conduct comprehensive psychosocial and bereavement risk assessments
• Facilitate referrals to community resources and financial assistance
• Provide counseling and support to patients/families on terminal illness and dying
• Participate in interdisciplinary team meetings and develop care plans
• Maintain clinical documentation and assist with discharge planning
