You'll join an interdisciplinary team to provide restorative and rehabilitative physical therapy services, maximizing participant independence and safety. You'll build long-term relationships with lower patient volumes and enjoy work-life balance. This role offers a holistic care environment with collaboration across disciplines.

🎯 What You'll Do

• Conduct physical therapy assessments in facility and home settings
• Develop participant care plans through IDT meetings
• Deliver and document interventions per care plans
• Supervise Therapy Assistants and implement Functional Maintenance Programs
• Provide training on therapeutic equipment to staff and families

📋 Requirements

• Bachelor’s degree in physical therapy
• Valid California Physical Therapy License or proof of eligibility
• CPR and First Aid certification within 180 days of hire

• Master’s or doctoral degree preferred
• Two years in clinical setting with frail or elderly population
• Outpatient, acute, inpatient, or skilled nursing experience
