Description

You will assess residents' health status, develop and revise dynamic care plans, administer medications and treatments, and collaborate with interdisciplinary team members to deliver effective nursing care in a rehabilitation setting. You'll handle changes in condition, respond to crises professionally, and maintain accurate documentation.

Requirements

• Graduate of accredited school of nursing with current RN license
• CPR/AED certification or willingness to obtain within 6 months
• Critical thinking and excellent communication skills
• Ability to stand/walk long periods, lift heavy objects, transfer patients (200+ lbs)
• Long-term care experience preferred

Responsibilities

• Assess residents' health, educational, and environmental needs on admission and ongoing
• Develop and revise dynamic care plans using interdisciplinary approach
• Administer medications, treatments, and IV fluids safely and timely
• Identify and communicate changes in condition using SBARR tool
• Monitor care delivery effectiveness and resolve problem areas via QAPI
