Description

You will lead our Assistive Care office, combining clinical leadership and team development to provide compassionate, reliable home health care across Middlesex and Somerset counties. You'll conduct client assessments, develop care plans, and mentor Home Health Aides.

Requirements

• Active RN license
• 2+ years of RN experience
• Home care or supervisory experience preferred

Responsibilities

• Lead, inspire and motivate others to provide exceptional care
• Provide clinical oversight for assistive care clients throughout the community
• Conduct client assessments and develop individualized care plans
• Supervise, mentor, and support Home Health Aides in the field
• Facilitate new hire orientation and conduct competencies on staff
