💼 About This Role

You'll join a multi-disciplinary health care team providing skilled nursing and rehabilitative care to clients in their homes, helping them regain independence. You'll develop and implement individualized care plans and apply innovative techniques to help clients reach their goals. This role offers a retention bonus up to $30,000.

🎯 What You'll Do

• Perform diagnostic tests and measurements of mobility, strength, and balance
• Develop and implement individualized care plans including therapeutic exercises
• Educate clients and family members on rehabilitative care activities
• Document patient care management and services using a touch pad tablet

📋 Requirements

• Physical Therapist license in Arizona
• Graduation from an APTA-approved program
• Solid computer skills
