💼 About This Role

You'll provide home health visits to adult and geriatric clients as part of a multi-disciplinary care team. Your core impact is enabling clients to remain safely at home through rehabilitative care and skilled nursing.

🎯 What You'll Do

• Perform diagnostic tests and measurements of mobility, strength, and balance.
• Develop and implement individualized care plans including therapeutic exercises.
• Educate clients and families on rehabilitative care and independent living.
• Document interventions and evaluations using a touch pad tablet.

📋 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by APTA or CAAHEP.
