💼 About This Role

You'll join the Transition Team at BAYADA Home Health Care, supporting infants, children, and adults transitioning from hospital or HICU® services to home care. You'll deliver quality care and provide extensive training to ongoing care teams, working with complex cases in partnership with CHOP. This role offers paid training and competitive pay.

🎯 What You'll Do

• Deliver quality care to clients in their homes
• Provide extensive training to the ongoing care team
• Support transition of care from hospital or HICU services
• Work overnight or weekend day shifts as scheduled

📋 Requirements

• Current LPN license in Pennsylvania
• NICU, PICU, ICU, or home care trach/vent experience
• Current CPR certification
• Ability to travel throughout Philadelphia County

✨ Nice to Have

• Experience with rare and complex pediatric diseases
• Teaching or training experience
• Spanish language skills
