💼 About This Role

You'll join Included Health to provide comprehensive virtual primary care to patients nationwide through video and phone visits, managing your own panel and fostering lasting relationships. You'll collaborate with a multidisciplinary team to deliver compassionate, patient-centered care that raises healthcare standards.

🎯 What You'll Do

• Provide high-quality virtual primary care through video visits including assessment and treatment.
• Manage a panel of patients across acute, chronic, and preventive needs.
• Collaborate with Care and Case Management team to develop care plans.
• Adhere to assigned schedules including some evenings and weekends.

📋 Requirements

• 4 years delivering primary care in ambulatory/outpatient setting.
• Active, unrestricted Nurse Practitioner and RN license in state of residence.
• Current board certification by ANCC or AANP.
• Ability to treat all ages across the lifespan.

✨ Nice to Have

• Experience with telehealth preferred.
• Ability to be licensed in additional states.
