💼 About This Role

As a Weekend Admission Nurse (RN) at 21st Century Home Health Services, you'll join the largest home health agency in San Francisco, conducting start of care admissions and providing skilled nursing care to patients in their homes. Your work directly contributes to our low hospital readmission rates and high patient satisfaction. 🎯 What You'll Do

• Conduct Start of Care (SOC) admissions and OASIS assessments
• Perform follow-up visits, medication reconciliation, and wound care
• Initiate and administer IV medications (antibiotics, diuretics)
• Document patient observations, interventions, and evaluations

📋 Requirements

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA Driver's License
• Experience conducting assessments and Start of Care (SOC) OASIS

• Experience with high acuity patients
• Previous experience with IV medication infusions
• Case management fundamentals
