Description

You will manage a patient panel and provide comprehensive primary care, including acute, chronic, and preventive care visits, working alongside a collaborative team with integrated virtual care and operational support. You'll deliver in-person and occasional telehealth care to a diverse population, including a large Spanish-speaking community, ensuring clear communication and coordination with specialists.

Requirements

Graduate of accredited Nurse Practitioner or Physician Assistant program
Current national certification (NP or PA)
Active California license or eligible for licensure prior to hire
Spanish speaking

Responsibilities

Manage a patient panel and provide comprehensive primary care including acute, chronic, and preventive visits
Deliver in-person care at clinic sites with occasional telehealth visits
Communicate clearly with patients from varied cultural and linguistic backgrounds
Collaborate with physicians, APPs, and clinical staff through daily huddles and care coordination
Perform in-office procedures within scope of training and credentials
