You'll provide exceptional medical care to geriatric patients in Senior Living Communities, performing home health visits. You will case manage and coordinate care while ensuring accurate documentation via electronic medical records. This role offers flexible scheduling and immediate openings.

🎯 What You'll Do

• Making home visits to clients in designated territories.
• Administering medication, wound care, treatments, and procedures.
• Monitoring clients' conditions and reporting changes to managers.
• Documenting observations and interventions in electronic medical records.

📋 Requirements

• Current LPN license in Massachusetts.
• Recent clinical experience as an LPN.
• Ability to perform home health visits in senior living communities.
