💼 About This Role

You'll lead proactive in-facility rounding across partner skilled nursing facilities in New Jersey, using AI-driven patient identification to detect early clinical deterioration. You will develop personalized care plans that integrate AI insights and evidence-based guidelines to reduce hospital transfers.

🎯 What You'll Do

• Conduct initial and routine patient assessments in facilities
• Review alerts from Circadia’s AI monitoring system
• Develop and update patient-specific care plans
• Coordinate with facility clinical teams for hand-offs

📋 Requirements

• Nurse Practitioner certification and license in NJ
• Active DEA number
• 2+ years NP experience with complex adult populations
• Current NJ Driver’s License
