Description

You will provide personal care to hospice patients with respect and dignity, working as part of an interdisciplinary team to deliver compassionate, person-centered care in a supportive faith-based ministry.

Requirements

• CNA/STNA certification in Ohio and active on State Tested Registry
• Minimum 1 year direct patient care experience in institutional or home care
• Valid driver's license and insured reliable transportation
• CPR required
• Understanding of Hospice philosophy and needs of terminally ill

Responsibilities

• Provide personal care with respect and dignity to patients and families
• Observe, record, and report patient's care, symptoms, emotional and physical status
• Assist with feeding patients as needed
• Attend in-services, IDT meetings, and QAPI activities
• Travel to patients between Lebanon, Middletown, and Springboro
