💼 About This Role

You'll provide rehabilitative care and skilled nursing to clients in senior living communities as part of a multi-disciplinary team. You'll develop and implement individualized care plans while conducting diagnostic tests and measurements. This role offers a $1,500 sign-on bonus for candidates with home health experience.

🎯 What You'll Do

• Perform diagnostic tests and measurements of mobility, strength, and balance.
• Implement wound prevention and treatment using evidence-based dressings.
• Develop and adjust individualized care plans with therapeutic exercises.
• Educate clients and families on rehabilitative care and activities.

📋 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from a CAPTE-approved physical therapy program.
