You'll assist clients and families with social and financial issues related to continuing care admissions or return to the community. You'll collaborate with the Community Care Coordinator to develop discharge plans and communicate progress toward the appropriate discharge location. This role supports clients in maintaining quality of life through their healthcare journey.

🎯 What You'll Do

• Assist clients and families with social and financial issues for continuing care.

• Communicate with clients and families about discharge progress.

📋 Requirements

• Graduation from a recognized degree program in Social Work or equivalent.
• Registration with Alberta Association of Social Workers.
• Valid Driver's License.
• Minimum 2 years experience in clinical and continuing care roles.
