Description

You will provide bedside care to Hospice patients, support families and caregivers, and work closely with the RN Case Manager in a community-based ministry setting.

Requirements

• Graduate of approved nursing program
• Valid Ohio LPN license
• CPR certification
• Minimum 2-4 years direct patient care experience in hospice or home care
• Valid driver's license and automobile insurance

Responsibilities

• Provide assistance in caring for Hospice patients
• Provide bedside care to patients
• Support families and caregivers
• Work closely with the RN Case Manager
• Comply with Hospice Medicare Conditions of Participation and Ohio Department of Health regulations
