You'll provide orientations, assessments, and training for patients and caregivers in a structured family caregiving program. You'll ensure quality care and patient independence while collaborating with care teams. Frequent fieldwork required in West Hartford and surrounding areas.

🎯 What You'll Do

• Perform home orientations and sign paperwork
• Conduct initial assessments during orientations
• Train caregivers and patients per case requirements
• Engage in interdisciplinary meetings with stakeholders

📋 Requirements

• 2+ years experience with frail or elderly disabled adults
• Valid driver's license with insured vehicle for state travel
• Working knowledge of ADL/IADL assessments
• Proficient in Microsoft Word, Excel, Outlook
