💼 About This Role

As a Discharge Planner at Charlie Health, you'll coordinate communication and discharge planning for clients in virtual behavioral health treatment. You'll provide treatment updates to referral sources, develop comprehensive discharge plans, and ensure seamless transitions to aftercare, directly contributing to expanding access to life-saving care.

🎯 What You'll Do

• Provide treatment updates to referral sources and build trusting relationships
• Develop clinically appropriate discharge plans for clients completing treatment
• Make accurate and timely referrals to aftercare resources
• Document case management contacts and communicate with treatment team
• Collaborate with internal stakeholders including clinical, admissions, and outreach teams

📋 Requirements

• Bachelor's degree in health sciences, communications, psychology, social work, or related field
• Minimum 2 years of relevant work experience in healthcare, preferably in case management, discharge planning, or referral relations
• Strong interpersonal, relationship-building, and listening skills
• Excellent written and verbal communication skills
• Proficient in Salesforce and Google Suite/MS Office
