💼 About This Role

You'll coordinate enhanced care management for high-need individuals involved with the justice system, connecting them to housing, mental health, and substance abuse services. Your work will reduce recidivism and support long-term stability. This role offers the chance to work within a therapeutic community and gain hands-on experience in reentry programs.

🎯 What You'll Do

• Conduct outreach to identify and engage eligible individuals for ECM services.
• Assess clients' medical, behavioral, and social needs and develop care plans.
• Coordinate care across providers and link clients to housing and treatment resources.
• Document services in electronic health records within 72 hours.

📋 Requirements

• Bachelor's degree in social work, psychology, public health, or related field, OR Associate's degree and 2 years' case management experience, OR high school diploma and 3 years' experience.
• Valid driver's license, current vehicle registration, and car insurance.
• Ability to travel locally to deliver services in the community.
