Description

You will be a member of an experienced hospice care team providing spiritual and emotional support to patients and families, developing spiritual care plans, and contributing to bereavement services. This role involves home visits in Chittenden, Franklin, and Washington Counties in Vermont.

Requirements

• Masters of Divinity degree
• At least one unit of CPE
• One year experience in spiritual counseling in healthcare setting, preferably hospice or palliative care
• Willingness to travel coverage area

Responsibilities

• Develop and adhere to client's spiritual care plan
• Provide spiritual and emotional support to patients and families
• Complete documentation accurately and timely
• Participate in bereavement services
