Description

As a home care Speech Language Pathologist, you will be an integral member of a multi-disciplinary team providing rehabilitative care and skilled nursing to clients at home. You will perform evaluations, develop treatment plans, and educate patients and families to promote health and independence.

Requirements

• Current license as a Speech Language Pathologist in Pennsylvania
• Certificate of clinical competence in speech from ASHA or equivalent
• Solid computer skills; EMR experience preferred
• Prior home care experience a plus but not required

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations for speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician orders
• Educate patients and family members in rehabilitative care
• Accurately document patient care using a touch pad tablet
