Description

You will perform personal care for hospice patients under RN supervision, observe and report patient status, and document care to ensure comfort and dignity at end of life.

Responsibilities

• Perform personal care needs after patient assignment
• Attend to patient requests promptly
• Report inability to perform tasks to Case Manager
• Observe patient and report observations
• Document observations and care performed
