💼 About This Role

You'll serve as a Health Navigator for a senior health member population (age 65+), establishing trusting relationships and coordinating care. You'll be an advocate for patients, removing barriers to care as part of a collaborative primary care team integrated with Amazon.

🎯 What You'll Do

• Room patients, take vitals, and perform screening assessments
• Coordinate post-visit care including referrals and specialist scheduling
• Monitor patient panel for preventive care and chronic disease management
• Provide patient education using motivational interviewing techniques

📋 Requirements

• 1 year experience in a patient-facing healthcare role
• Proficiency in computer technology and navigating multiple software systems
• Collaborative work in multi-disciplinary teams
• Strong written and verbal communication skills
