Description

You will make home visits to provide speech therapy evaluations and interventions for adult and geriatric patients. You'll work as part of a multi-disciplinary team to help clients remain safely at home, using a touch pad tablet for documentation.

Requirements

• Current Speech Language Pathologist license in Vermont
• Certificate of Clinical Competence in Speech from ASHA or equivalent
• Solid computer skills
• Prior home care experience is a plus but not required
• Prior experience with electronic medical records preferred

Responsibilities

• Perform home visits for speech therapy evaluations and interventions
• Develop treatment plans based on evaluations and physician orders
• Educate patients and families on rehabilitative care
• Document patient care using a touch pad tablet
• Participate in multi-disciplinary care plan meetings
