💼 About This Role

You'll perform clinical evaluations for an Adult Foster Care program, assessing patient needs to ensure quality care and compliance. You'll conduct monthly home visits to monitor patient progress and support caregivers.

🎯 What You'll Do

• Provide support to caregivers and members per Adult Foster Care scope.
• Assess and reassess client needs and update the plan of care.
• Coordinate care with physicians, RNs, and other disciplines.
• Complete timely and accurate clinical documentation after visits.

📋 Requirements

• Registered Nurse license by Georgia Board of Registration in Nursing.
• Minimum 2 years of clinical experience.
• Minimum 1 year experience with elders or adults with disabilities.
• Valid driver's license with insured vehicle for travel within Georgia.

• Bilingual in Spanish
• Experience with CRM (Salesforce a plus)
