Description

You will provide personal care and related services to home health patients in their residences, nursing homes, or inpatient units, following an established care plan under the supervision of a registered nurse or therapist. Your role involves observing patients, reporting changes, documenting care, and ensuring a safe environment while traveling locally throughout the greater West Liberty, Ohio area.

Requirements

• High school diploma or equivalent
• Completion of state-approved CNA/STNA training program
• Active Ohio Nurse Aide Registry certification
• Minimum 75 hours of home health aide training per 42 C.F.R. Section 484.80(b)
• Pass written competency test per 42 C.F.R. Section 484.80(a) before independent work

Responsibilities

• Perform personal care activities (hygiene, mobility, elimination) per care plan
• Maintain safe environment and use equipment properly
• Monitor vital signs, intake, and body functioning; document and report changes
• Assist with home maintenance (light housekeeping, meal preparation)
• Manage caseload and deliver care cost-effectively while meeting service standards
