Description

You will provide rehabilitative care to adult and geriatric patients in their homes, performing evaluations and developing individualized intervention plans to improve their functional independence and safety.

Requirements

• Current North Carolina Occupational Therapist license
• Graduation from an occupational therapy program accredited by AMA and AOTA
• Minimum one year of recent work experience as an OT
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations of functional status and ADL/IADL performance
• Develop individualized intervention plans to achieve favorable outcomes
• Assess home environment for hazards and recommend equipment adaptations
• Educate patients and families on rehabilitative care
