💼 About This Role

You'll serve as a Transitional Care Manager, building relationships between referral sources and the company while educating patients about home health care. You'll use clinical knowledge to manage referral intake and coordinate the transition to home care. This role involves daily travel to area hospitals.

🎯 What You'll Do

• Develop relationships with referral sources and physicians
• Market and sell BAYADA services to healthcare professionals
• Review medical documentation and determine home care eligibility
• Coordinate with service office to set up ordered services

📋 Requirements

• Current RN or LPN license in good standing
• At least 1 year experience in healthcare
• Knowledge of Medicare COPs and program requirements
