You'll provide patient-centered care to residents in skilled nursing facilities as part of a mission-driven care community. You'll build long-term relationships with patients, strengthening continuity and making a lasting impact. Flexible scheduling and autonomy are supported by intuitive technology.

🎯 What You'll Do

• Conduct daily rounding in skilled nursing facilities
• Prescribe medications and treatment plans
• Address acute and emergent issues
• Communicate with facility nursing staff via phone or secure messaging

📋 Requirements

• Active U.S. license and board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Qualified specialty in Internal Medicine, Family Medicine, or related field
• Ability to perform essential functions with or without accommodation
• Pass criminal background check and drug testing

• Experience in geriatric or post-acute care
• Experience with skilled nursing facility workflows
