💼 About This Role

You'll serve as a clinical advocate for cancer patients, conducting telephonic assessments and coordinating care transitions. You'll ensure members receive timely follow-up and support, reducing readmissions and improving outcomes. This role combines high-touch human support with technology-driven care coordination to make a real difference.

🎯 What You'll Do

• Conduct telephonic clinical assessments and medication reconciliation
• Coordinate discharge plans with hospital case managers
• Monitor member progress and provide regular updates to the healthcare team
• Address social determinants of health needs like food and transportation

📋 Requirements

• Active unrestricted compact RN license
• 5+ years of nursing experience with 3 years oncology nursing or case management
• Certified Case Manager (CCM) credential
• Comfort with video chatting, Google Suite, Slack, EHRs
