Description

You will provide home health visits to adult and geriatric clients in Nassau and Northern Duval counties, performing OASIS start-of-care assessments, following care plans, and documenting with a touch pad tablet. Your work helps clients remain safely at home with comfort and dignity.

Requirements

• Current RN license in Florida
• Minimum 1 year recent clinical medical/surgical experience
• Valid driver's license and ability to travel to cases
• Solid computer skills; EMR experience preferred
• Oasis/home health experience strongly preferred

Responsibilities

• Follow designated care plans and perform home visits in assigned territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to management
• Perform OASIS start-of-care assessments and execute doctors' orders
• Document observations and interventions using electronic tablet
