You'll provide psychosocial support to residents and families in a senior living community. You'll create an environment that enhances quality of life and promotes community health. This part-time role offers flexible hours and the chance to work with a non-profit organization.

🎯 What You'll Do

• Complete psychosocial assessments and care plans for residents
• Lead grievance committee and address resident concerns timely
• Coordinate discharges and care conferences
• Provide support across Independent Living, Assisted Living, and Nursing

📋 Requirements

• OH Licensed Social Worker license required
• 2 years related work experience in a nursing home setting
• Post-secondary education preferred

• Experience with MDS completion
• Knowledge of Medicaid Waiver process
• Ability to supervise social work interns
