You'll provide home health visits to adult and geriatric clients, performing diagnostic tests and developing individualized care plans. Your work enables clients to receive medical care at home.

🎯 What You'll Do

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements
• Develop and implement individualized care plans
• Educate clients and family members on rehabilitative care

📋 Requirements

• Current Pennsylvania Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from an APTA-approved program
