Description

You will provide high-acuity one-on-one care to adult and pediatric clients in their homes across all five NYC boroughs, supporting new case starts, urgent callouts, and complex care needs as part of a dynamic critical care team.

Requirements

• Active LPN license in New York
• Graduation from an accredited nursing program
• 1+ year trach/vent experience (home care preferred)
• Comfortable working with both adult and pediatric clients
• Willingness to travel to all 5 boroughs

Responsibilities

• Deliver high-acuity care to adult and pediatric clients
• Support new case openings and emergency coverage needs
• Precept and mentor new nurses
• Collaborate with multidisciplinary care teams
• Educate and support families and caregivers
