💼 About This Role

You'll provide home health visits to adult and geriatric patients as part of a multi-disciplinary team. Your care helps patients remain at home while receiving rehabilitative and skilled nursing support. This per visit opportunity offers flexibility and comprehensive benefits.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform evaluations for speech, language, communication, or swallowing difficulties.
• Develop and revise treatment plans based on evaluations and physician orders.
• Educate patients and families on rehabilitative care and independent living.

📋 Requirements

• Current Speech Language Pathologist license in Colorado.
• Certificate of Clinical Competence in Speech from ASHA or equivalent.
• At least 1 year of experience as an SLP.
