Description

As a Medical Social Worker, you will be an integral member of a multi-disciplinary healthcare team providing home health and hospice care. You will conduct clinical social evaluations, identify community resources, and advocate for clients to help them remain at home.

Requirements

• Master's degree in social work with at least 2 years community-based experience
• Accreditation by Council on Social Work Education
• Licensed in State of Hawaii
• Solid computer skills; prior experience with EMR preferred
• Ability to travel to assigned cases

Responsibilities

• Assist team in understanding social/emotional factors related to clients' health problems
• Provide clinical social evaluations and plan appropriate interventions
• Identify community resources to address practical and environmental needs
• Act as advocate for client and family to navigate community system
• Participate in developing total plan of care with multidisciplinary team
