Description

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients, allowing them to remain at home. You will perform evaluations, develop individualized intervention plans, and educate patients and families to promote independent living.

Requirements

• Current New Jersey Occupational Therapist license.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.

Responsibilities

• Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans.
• Evaluate home environment for hazards or barriers to independent living and identify equipment needs and environmental adaptations.
• Participate in the multidisciplinary care plan and continually assess/revise the occupational therapy care plan.
• Educate patients and family members in rehabilitative care and activities to promote health, safety and independent living.
