You'll join a multi-disciplinary team providing home health visits in senior living communities. Your core impact is enabling clients to remain at home through rehabilitative care. This role offers a $5,000 sign-on bonus for full-time.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility and strength.
• Develop and implement individualized care plans.
• Educate clients and families on rehabilitative care activities.

📋 Requirements

• Current Pennsylvania PT license.
• Minimum one year of recent PT experience.
• Graduation from an APTA-accredited program.
• Ability to work independently and manage time effectively.
