🎯 What You'll Do

• Collect clinical data to support care planning
• Provide care plan support and transition interventions
• Facilitate care coordination and health education
• Adjudicate referrals using evidence-based criteria

📋 Requirements

• Diploma from an accredited PT/PTA or OT/COTA program
• Active Physical Therapist or Occupational Therapist license in Florida
• Minimum 2 years of experience in medical management clinical functions
• Working knowledge of FIMS and NCQA standards
