Description

You will provide compassionate home health visits to adult and geriatric clients in Philadelphia, following care plans, administering medications, performing wound care, and monitoring conditions. This role offers the opportunity to make a meaningful impact while working a weekend schedule.

Requirements

• Current RN license in Pennsylvania
• At least one year of recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to travel to cases
• Solid computer skills; EMR experience preferred

Responsibilities

• Follow designated care plan per patient's needs
• Make home visits in assigned geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes
• Document observations, interventions, and evaluations using touch pad tablet
