Description

In this role, you will use your lived recovery experience to empower patients with substance use and co-occurring mental health disorders. You will help identify barriers to recovery, coordinate care, educate on community resources, and advocate for patients, while collaborating with a care team and maintaining documentation.

Requirements

• 2+ years professional experience in peer support
• 1+ years navigating OUD treatment resources and referrals
• Active peer certification in state of residence
• Minimum 2 years recovery from substance use or co-occurring mental health disorders
• NCPRSS certification expected within 9 months of hire

Responsibilities

• Conduct patient engagement and assist in identifying recovery barriers
• Assess patient progress and develop recovery plans
• Educate on community resources and facilitate higher-level care placement
• Provide social/emotional support and non-emergency resources
• Maintain communication with patients and care team
