Description

You will lead provider network strategy and operations for Habitat Health's PACE model, overseeing contracting, provider relationships, and network adequacy to ensure comprehensive, compliant care for older adults.

Requirements

Experience in provider network management, contracting, or managed care
Knowledge of PACE or value-based care models preferred
Strong negotiation and relationship management skills
Ability to translate organizational strategy into operational execution
Supervisory or leadership experience

Responsibilities

Draft and negotiate provider contracts, amendments, and single case agreements
Lead network strategy development and execution for assigned markets
Oversee provider recruitment, onboarding, and ongoing relationship management
Ensure regulatory compliance and integration of providers into clinical workflows
Support market expansion and network adequacy planning
