💼 About This Role

You'll join a multi-disciplinary health care team providing skilled nursing and rehabilitative care to clients in their homes. You'll make home visits and provide clinical social evaluations to help clients remain at home. This role offers the opportunity to work with a leading home health care provider.

🎯 What You'll Do

• Make home visits to clients in designated territories.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for clients and families.
• Advocate for clients and families to navigate the community system.

📋 Requirements

• Master's Degree in Social Work
• Licensed Social Worker (LCSW or LICSW) in Massachusetts
• One year of experience as a Licensed Social Worker
