Description

As a Weekend Per Diem Home Health Physical Therapist, you will provide skilled rehabilitative care to geriatric patients in senior living communities, helping them maintain their independence. You'll develop individualized care plans, perform assessments, and collaborate with a multidisciplinary team.

Requirements

• Current Physical Therapist license in Massachusetts
• Minimum one year of recent clinical experience as a licensed PT
• Prior home health experience required

Responsibilities

• Make visits to clients in senior living communities
• Perform diagnostic tests and measurements (mobility, balance, strength, etc.)
• Develop and implement individualized care plans including manual therapy and gait training
• Educate clients and family members on rehabilitative care
• Document observations and evaluations using a touch pad tablet
