Description

You will provide personal care and comfort to hospice patients under the supervision of a registered nurse, observing and documenting patient conditions to promote quality end-of-life care.

Responsibilities

• Perform personal care needs after patient assignment
• Attend to patient requests promptly
• Report inability to perform tasks to Case Manager immediately
• Observe patient and report findings
• Document observations and care performed
