Description

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients, allowing them to remain at home. You will make home visits, perform diagnostic tests, develop care plans, and educate clients and families.

Requirements

• Current Maryland Physical Therapist license
• One year prior clinical experience as a licensed Physical Therapist
• Current home health and Oasis experience for per diem
• Experience with adult and geriatric clients

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of joints, balance, etc.)
• Implement integumentary protective interventions to prevent wounds and promote repair
• Develop and implement individualized care plans including manual therapeutic exercises and gait training
• Educate and instruct clients, family members, or other representatives in rehabilitative care
