Description

You will conduct in-home assessments and functional evaluations, deliver evidence-based recommendations for home modifications, and educate clients and caregivers on fall prevention and safe mobility techniques. Collaborate with the Director of Clinical Operations and maintain accurate documentation. Your work helps older adults and individuals with disabilities live safely and independently.

Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Reliable transportation and valid driver's license
