💼 About This Role

You'll create and implement care plans for home health patients in the Windsor, CA area. You'll case manage patient episodes with an interdisciplinary team to prevent re-hospitalization. This role offers autonomy and flexible scheduling with a supportive team culture.

🎯 What You'll Do

• Perform full patient assessment and develop care plans
• Case manage patient episodes with interdisciplinary team
• Administer IV medications and manage complex treatments
• Collaborate with physicians and document clinical notes

📋 Requirements

• Current CA RN license and BLS/CPR certification
• Valid CA Driver's License and reliable transportation
• Willingness to work Saturday or Sunday
• High level of clinical knowledge in home health care

✨ Nice to Have

• Experience with high acuity patients
• Previous experience with IV medication infusions
• Case management fundamentals and interdisciplinary teamwork
